EPLD PROGRAM PROPOSAL

If you’d like to present a program at Eureka Public Library, please read the information from our Program Agreement below and then fill out the online Program Proposal Form.

​Please note: Planning is typically done 3-4 months prior to the program date. Due to the number of program proposals received, the Library is unable to individually respond to each inquiry. All submissions are reviewed. If accepted, a Library staff member will contact you and provide you with a copy of the Program Agreement to sign. Any materials dropped off are considered a donation.

Program Agreement

The EPLD Program Agreement establishes the mission and intent of library programming initiatives as well as stipulations on program protocol.
  • All EPLD library programs are library initiated and the library reserves the right to revise program arrangements scheduled if necessary and to preempt established programs upon reasonable notification to presenter.
  • Presenter is an independent contractor and not an employee of EPLD.
  • The program presenter will provide a description of the program to EPLD at least six weeks prior to the program date.
  • EPLD may use Presenter’s name, photographs, or likenesses in advertising and publicizing the program. EPLD will provide advertising through their calendar, website, newsletter, flyers, and press releases.
  • Program presenter and EPLD will establish program details, including class fee, required supplies, tech needs, room arrangement, registration deadlines, and maximum/minimum audience number six weeks prior to the program date. EPLD will be expected to meet all established requirements; if a conflict arises, notice will be given to the program presenter.
  • Payment in full will be issued via check on the day of the program (if in person) or mailed after the program has taken place (if virtual). No payment will be made in advance.
  • Presenter will inform EPLD as soon as possible if it becomes necessary to replace essential personnel for the program and EPLD reserves the right to cancel under such circumstances.
  • Presenter must notify EPLD of program cancellation a minimum of 3 days prior to the date of the program. If possible, EPLD will try to work with the Presenter to reschedule the program.
  • EPLD will contact the program presenter prior to or on the registration date to give an update on registration status and, if necessary, make the decision to cancel the program. EPLD reserves the right to cancel the program due to low registration numbers up to 72 hours in advance of the scheduled time/date and the Presenter will not hold the library responsible for payment unless notification of cancellation is not provided within 72 hours of the event’s scheduled time/date.
  • The program presenter will be expected to arrive 15—30 minutes prior to the start of the program. If they will arrive less than 15 minutes before the start, they are requested to call the library to confirm arrival time.
  • Programs must end, audience members released, and presentation materials removed by library closing time.
  • If the program presenter has a personal business that could benefit from the program being given, EPLD asks that the program content be educational/informational only, no sales take place, and no business/sales literature is given without direct request. Business cards or brochures may be made available after the presentation, if an audience member asks. For authors, book sales and signings are allowed.
  • EPLD will not share registration information, including names, phone numbers, emails, or addresses of audience members, nor may it be solicited.
  • Presenter participates at their own risk and assumes all risk of damage or loss to their merchandise, personal property, and personal effects.
  • Presenter releases and discharges EPLD and its Trustees, Officers, Employees, or Agents from any and all claims, causes of action, losses, or other damage resulting from, arising out of, or relating in any way to their program.
  • Presenter shall indemnify EPLD and its Trustees, Officers, Employees, or Agents and save them harmless from losses, damages, or claims in connection with their program.
  • EPLD reserves the right to make in-house announcements and/or sponsorship announcements prior to and/or following the program.
  • Any legal action or arbitration arising from this Agreement shall be held in the County of Woodford, State of Illinois.
