The Eureka Public Library District (EPLD) wants to learn about any issues which prevent or make it difficult for people to visit our facility, check out our collections, attend programs, or utilize our services.  If you or someone you know has difficulty accessing the library, please describe on the form so that we can evaluate possible improvements.
